What you get

A library, not an empty page

Nineteen forms you can send and twenty session-note templates, in each of English and Greek. Adult, child and couples intake, informed consent, telehealth and GDPR consent, cancellation policy, release of information, and nine client worksheets. Use them as they are or edit any of them.

No account for the client

They tap a link, confirm their date of birth, and start. It works on any phone, saves as they go, and survives being interrupted. That last part is what actually gets forms finished.

Answers reach the record

Phone number, address, occupation, relationship status: you see what changed, tick what you want, and it updates. Nothing is written without you confirming it.

Consent that holds up

The terms are on the same page as the box that accepts them, signed with a real signature, stamped with the time it was submitted, and exportable to PDF or Word.

Send it without being asked

One sentence: when you add a new client, send the intake form. When a session finishes, send the feedback form. It is optional. Plenty of practices never turn it on.

Greek and English throughout

Not a translated interface with English forms inside it. Every template exists properly in both languages, and your client gets theirs.

Three steps, and two of them are not yours.

  1. 1. Pick a form and send it

    Choose from the library or one of your own. Send to one client or twelve at once, with a due date if you want one.

  2. 2. They fill it in

    A link by email or text. They confirm their date of birth and answer at their own pace. It saves as they go, so a half-finished form on the bus is not lost.

  3. 3. You read it, and the record updates

    It arrives on their profile with the answers laid out beside the questions they were asked. Anything that belongs on their record is offered to you to confirm.

Counts are per language. Everything listed exists in both English and Greek.

What this is not

There is no scoring. We do not ship PHQ-9, GAD-7 or any other measure that produces a number a clinician interprets. A form here collects answers. Reading them is your job, and nothing is analysed, flagged or interpreted for you.

AyloCare is also not a monitoring or emergency service. A submitted form notifies you when it arrives, but nobody is watching it on your behalf and it is not checked for risk. Tell your clients how to reach emergency help.

Questions

No. They tap the link and confirm their date of birth, which is what proves it is them. There is nothing to install and no password to forget. If they do have a portal account, the same forms appear there too.

Both. Every template is editable, and you can build your own from scratch in the same editor you already use for notes. Short answers, long answers, multiple choice, dates, signatures. You also choose which answers map onto the client record.

They are written from general clinical practice and record what matters: the terms shown on the same page as the box accepting them, a real signature, and a timestamp with the IP address the form was submitted from, exportable as PDF or Word. They are a solid starting point, not legal advice. Have your own terms reviewed if that matters to you.

They are stored encrypted, visible only to the practice, and shown to you beside the questions exactly as your client saw them. You can export any submission, and anything that belongs on the client record is offered to you to confirm rather than written automatically.

No, and it is off until you set it up. We ship no rules at all. A rule you do create applies from that moment on. Switching on "send intake to every new client" never sends anything to the clients you already have.

No. Forms are included wherever templates are, on the same plan, with no per-form or per-client cost. The library is free to use and free to edit.
